Chuck Bongiovanni, MSW, MBA, CSA, CFE
As CarePatrol’s Chief Executive Officer, Chuck is responsible for CarePatrol’s national strategic vision, as well as leading the company’s national expansion and services strategy development. CarePatrol began franchising in 2009 and has grown to more than 150 offices nationwide. providing personalized expert placement services to seniors & families.

Chuck is a nationally recognized expert, speaker, author, and Founder of The Community Integration Model, a specialized discharge planning model for hospitals, ACOs, Bundles, and Healthcare Plans to reduce readmissions and more importantly, reduce Medicare Spending Per Beneficiary. The Model is a powerful solution for Bundled Payment Organizations, ACO’s and Health Plans.
He is currently the President of two non-profit organizations, including The National Placement and Referral Alliance (NPRAlliance.org), which is the national trade association for the Referral and Placement Industry, He is also the President of The National Post-Acute Care Continuum (NPACC.org),
He currently serves as a Board Member of the Reflections Hospice and Palliative Care’s non-profit, Sunrise Points of Light Foundation. He has served on the Boards of The Caregiver Resource Institute, and The Professional Association of Senior Referral Specialists in Arizona.
Chuck’s vision for the Referral & Placement Industry is business environment which fosters a “Fair Marketplace For All” while giving “Families The Right Of Choice” of Referral and Placement service providers.
Scott Fischer is co-founder and Owner of Options for Senior Living, an Arizona senior referral company. One of the founding members of PASRS, Scott has served the healthcare industry for over 25 years. Scott’s vast nursing experience includes nursing administration, nursing education, mental health nursing and home health nursing. Scott has dedicated the past 15 years of his career to the care and service of seniors, particularly in the area of senior mental health with an emphasis on seniors suffering from dementia
Frank Sampson, CSA
Frank is the CEO of Senior Care Authority Franchise LLC. Prior to starting Senior Care Authority, Frank was a veteran of the travel industry for 34 years.
Most recently, he operated a company specializing in the design of customized luxury vacations around the globe. Prior to that, Frank was the founder and president of InHouse Travel Group in Chicago that he franchised in 1995 and grew to 200 franchises within a 3-year period. The company was listed as one of the “Fastest Growing Franchises” in Entrepreneur Magazine. He sold the business to Uniglobe
Frank has had a passion for senior care since having to care for family members and personally experience the challenges that face families today. After several years of researching the health care industry, he began Senior Care Authority, which has already helped many families throughout the United States. The company also is franchising the program throughout country.
In addition, he hosts a twice-weekly podcast and radio show called “The Aging Boomers” to help educate family caregivers and seniors on issues facing them while providing important resources nationwide. The show appears on iTunes, iHeartRadio, Spreaker and other sites.
Frank also holds the designation of Certified Senior Advisor.
Linda has been assisting families with senior placements since 2007. She is the founder of Clear Choice Senior Services, assisting seniors and families to find the best options for assisted living when they can’t safely remain at home. Her credentials include; BA in Biology, Certified Senior Advisor (CSA), Certificate in Gerontology, Residential Care Facility for Elderly (RCFE)Administrator.
She is also a past and current Board member of North Orange County Senior Collaborative (NOCSC) service organization and the Orange Senior Center. She serves as NOCSC’s Chairman of the Elder Abuse Awareness work group, and representative to Ageless Alliance and FAST (Financial Abuse Specialists Team). She served from 2009 to 2016 as Chair of OC Senior Services Roundtable in Orange, is a past member of Hospice Care of CA’s Professional Advisory Board, and is founding member and President of Southern CA Association of Senior Referral Agencies.
In 2015, she was featured in an article in the Fullerton News Tribune and Orange County Register as a recipient of Fairhaven Memorial’s Oliver Halsell Care Award, which pays tribute to individuals who have demonstrated exceptional care in their profession and community, improving the lives and spirits of those they help. In June 2016 she received a Lifetime Achievement in Senior Services award from the Orange County Board of Supervisors for her work on elder abuse awareness and prevention. Linda is a founding member of Senior Care Hero Awards annual event and currently serves on their Council of Advisors.
In 2015, 2016 and 2017 she testified in Sacramento at several committee hearings regarding legislation to license and regulate senior referral agencies. She helped to compose amendments for referral agency minimum standards, including training requirements, mandated reporter for elder abuse, anti-kickback provisions, and other consumer protection requirements which were later adopted into proposed legislation.
CFO and Chief Strategy Officer
Mark is a performance-driven executive with a reputation for delivering business strategies and solutions that improve operating margins. As the CFO and Chief Strategy Officer of Assisted Living Locators, Mark ensures that the company is properly positioned to continue to capture a growing share of this dynamic industry segment.
Mark has helped guide this award-winning franchise system to three consecutive rankings in Entrepreneur Magazine’s “Franchise500®”, as well as recognition in Franchise Business Reviews “2019 Top Franchises” report, and has been directly responsible for managing profitability, operations and key business and personnel decisions as the former COO of the organization. Under his guidance, the organization recently completed a senior executive transition plan designed to support the company’s long-term strategy for both growth and profitability.
Mark is personally responsible for managing all financial and strategic aspects of the business, has lead responsibility for all franchisor/franchisee communications, and plays a key role in supporting the company’s franchise growth opportunities and guiding its national expansion.
Mark has an economics degree from Cornell University, a law degree and has received numerous awards for his outstanding work, including Marshall Reddick’s Rising Star Award.
